Counterparty information

The broker, platform, or counterparty domain. Names of individuals you communicated with. Telegram handles, WhatsApp numbers, or email addresses used.

Documentation

Screenshots of communications, account dashboards, balance views. KYC documents you submitted to the counterparty. Account statements if accessible.

Timeline

A written sequence of events — when funds were sent, when withdrawal was requested, when issues began. Even rough dates help.

Loss amount

Approximate USD value at time of loss. Original cryptocurrency amounts where applicable. Any recovery fees already paid (if any) and to whom.
